Every time patients interact with the

to repeat the same exhaustive list of demographic information,
insurance information, and medical history. Usually, they
are required to repeat their medical history multiple times
as they are interviewed by different physicians and nursing
staff in the same medical facility. This information is then
typically entered into different sections of a handwritten
chart. If a patient changes primary care physicians, goes
across town to a different medical facility, is referred to
a specialist, or even returns to the same medical facility
a few weeks later, the whole process has to be repeated.

a complete medical history into simple steps: allergies, current
medications, etc., patients are able to complete a comprehensive,
uniform medical history on their own. A patients information,
including demographic and insurance information, is then securely
stored. Once a patient completes an initial registration and
medical history, the information only needs to be updated
if any changes occur, never completely redone. Furthermore,
patients can designate which practitioners and medical facilities
have access to their information, regardless of what healthcare
system a physician or medical facility is a part of.
