Having an up to date preop H&P in the patient’s record before the start of a surgery or procedure is a mandated requirement of all credentialing agencies such as AAAHC and JCAHO.  Frequently however, this can mean waiting for the surgeon or other physician to take a brief history from the patient at the bedside which is then handwritten in the patient’s chart.
