Several healthcare organizations including the Institute of Medicine, have accurately chronicled how a disproportionate number of medical “mistakes” occur during the transfer of a patient’s care from one healthcare team to another.
A prime example of this is when a patient leaves the OR and is admitted to the PACU. Usually, a very rapid verbal report is given by the anesthesia team and the OR nurse to the PACU nurse, who is simultaneously applying monitors and orienting the patient to the PACU. Needless to say, in this often very hectic and fast-paced environment, the opportunity for the PACU nurse to not hear a critical piece of information is high.
